Responsible for data collection and abstraction for various quality/safety/improvement projects and publically reported measures. Performs a wide range of duties involving the collection, abstraction, and recording of clinical patient data following department guidelines and standards. Reviews cases as needed based on current performance improvement or abstraction focus. Relays information to clinicians or improvement teams based on data collection/abstraction in order to improve performance. Is a member of performance improvement or service line teams as appropriate.I. Major Responsibilities: 1. Performs concurrent and retrospective medical record review to: 2. Identifies and retrieves data (per specific project requirement) from electronic record or other data source based on abstraction needs. 3. Coordinates data collection for abstraction along with any outside vendors; completes appropriate forms and enters data into electronic data base(s) and may submit to external sources. All responsibilities are essential job functions. II. Position Qualifications: License/Certification/Education: Preferred: Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. The position focuses on disease specific abstraction, analysis and improvement working closely with the Stroke Coordinator and other Quality, Safety and Regulatory Team members.
