Job Overview: The Advanced Heart Failure Nurse - RN plays a central role in the outpatient management of patients with complex cardiac conditions, including advanced heart failure, cardiomyopathies, and hypertrophic cardiomyopathy - HCM, Working closely with cardiologists, APPs, and the multidisciplinary heart failure team, the RN provides comprehensive clinical support, patient education, medication management, and ongoing care coordination to optimize patient outcomes and quality of life, This role is ideal for an RN who is looking for a fast-paced, highly specialized environment and enjoys building strong long-term therapeutic relationships with patients and families, Qualifications:
