Responsibilities:

The person in this position will build and manage relationships with community partners and organizations to support classes and related initiatives in the community, including Presbyterian community kitchens. Provide individual and group diabetes self-management education and support services and/or medical nutrition therapy, delivering patient-centered care.

  • Responsible for management and coordination of Community Health healthy eating and active living class programs and initiatives.
  • Interface with PHS providers, dietitians and other health care team members to facilitate and manage referrals to programs.
  • Interface with community partners, contractors, coalitions, and organizations to align with community and organization goals.
  • Collaborates internally and with community-based organizations to develop and host health promotion events for the community.
  • Attend related internal and external meetings and events as necessary.
  • Train and manage staff and partners implementing programming in Presbyterian teaching kitchens.
  • Assists in identifying funding opportunities, including writing and managing grants to scale and sustain nutrition and cooking programs.
  • Responsible for development and implementation of healthy eating and active living programs
  • Utilize data, community health assessments and implementation plans to lead development of evidence-based culinary and nutrition curriculum and programs for patients, members, and community focused on health promotion, chronic disease prevention and management with the goal of facilitating lifestyle change.
  • Accepts responsibility and accountability for providing competent, ethical, client-centered nutrition and dietetics services.
  • Lead development of nutrition education strategies with Food Farmacy.
  • Procure necessary supplies for classes, to include but not limited to fresh and frozen foods, cooking equipment and sanitary supplies. Manage inventory and mandatory community kitchen safety and inspection requirements, including staff training.
  • Coordinate class schedule with partners and contractors.
  • Manage maintenance of community kitchens with appropriate PHS personnel.
  • Develop nutrition education materials and recipes, including nutritional analysis. Evaluates nutrition education materials provided for literacy, accuracy, effectiveness, and adherence to standards of nutrition care.
  • Dietitian responsible to fulfill responsibilities to represent PHS in the Teaching Kitchen Collaborative, including attend meetings and interact with the TKC collaborative fellow institutions to help elevate and provide medical professionals with state of the science updates relating to nutrition science, exercise and movement, mindfulness, and health coaching, with the intention to shape and drive next-generation strategy and collaborative research on best practices of integrative lifestyle transformation.

Qualifications:

  • Bachelors degree in Nutrition or related discipline required. Masters degree in nutrition or related discipline preferred.
  • Registered by the Academy of Nutrition and Dietetics (AND) required.
  • LDN (Licensed Dietitian/Nutritionist) required.
  • Certified Diabetes Care and Education Specialist (CDCES) credential preferred.
  • Minimum of 3 years experience as a dietitian, preferably in a community or clinic setting.
  • Culinary experience desired.
  • Ability to work in a culturally diverse, patient-centered community-based environment.
  • Excellent oral and written communication skills required, including by phone and group presentations.
  • Ability to lead and work as part of a professional team and to collaborate effectively internally and with external partners.
  • Ability to use various standardized word processing and spreadsheet software required.
