We are seeking a dedicated Community Health Worker (CHW) who provides patient centered support in the ambulatory care setting under indirect supervision. In this role, you will collaborate with Care Managers, Medical Providers, and the broader health care team to deliver high quality support aligned with established standards of care. You will also connect patients to community based programs that enhance their health, well being, and access to essential services through education, coordination, and resource navigation.

Responsibilities: As a Community Health Worker, you will play a vital role in supporting patients, strengthening care coordination, and addressing social determinants of health. This position requires a compassionate, resourceful, and communityfocused individual who can build strong relationships, empower patients, and serve as a knowledgeable resource for both patients and the care team.
- Participate in presentations to clinics and hospital staff.
- Seek and complete training opportunities related to CHW specialty areas and/or CHW Core Competency training to build subjectmatter expertise.
- Cofacilitate health and wellness classes, workshops, and trainings.
- Assist with orientation and mentoring of new team members.
- Participate in local, regional, or state community health worker events and meetings.
- Screen patients for social determinants of health and connect them to appropriate community resources.
- Use motivational interviewing and other engagement techniques to support patient and family selfmanagement.
- Act as an advocate for patients and families.
- Educate patients, families, providers, and care team members on social determinants of health.
- Build relationships with internal and external stakeholders, including CHWs and peers, to support community health improvement.
- Continuously expand knowledge of community resources and available services.
- Facilitate patient access to community resources, including social service agencies, and assist with completing applications.
- When applicable, initiate closedloop referrals for eligible programs.
- Work collaboratively and effectively within a team, establishing positive, supportive relationships with participants and providing feedback to team members.
- Build and maintain positive working relationships with clients, providers, case managers, social workers, agency representatives, supervisors, and office staff from diverse cultural and socioeconomic backgrounds.
- Work to reduce cultural and socioeconomic barriers between clients and institutions.
- Perform other functions as required.
Qualifications:
- High school diploma or GED equivalent
- One year of experience preferred interacting with community members or in community/public health services.
- CHW Certification required as training is available.
- Must be able to speak language of the population served.
Working Conditions
- Comfortable working in an Emergency Department and/or clinical setting, including Behavioral Health.
- Can work in both clinical and community-based settings, including clients' homes
