Your Role: As a Case Manager, you will provide consumer-centered, recovery-oriented support that helps individuals thrive in their daily lives and communities. You will assess needs, develop and monitor treatment plans, advocate for services, and ensure compliance with all program and legal requirements. What You'll Do:
- Engage & Support - Provide recovery-oriented, consumer-centered care in accordance with professional standards.
- Assess Needs - Conduct comprehensive evaluations of consumers' physical, psychological, and social needs; coordinate specialized services as needed.
- Develop & Implement Plans - Collaborate with consumers to create, implement, and update individualized treatment plans, including counseling, community resources, vocational services, and home visits.
- Monitor Progress - Continuously review and adjust treatment plans to ensure they meet evolving consumer needs.
- Advocate for Services - Assist consumers and families in accessing benefits such as medical assistance, transportation, and educational resources.
- Teach Daily Living Skills - Provide education and support in budgeting, money management, medication management, and independent living skills.
- Ensure Compliance - Complete and submit required documentation within established timeframes (including TCM documentation within 24 hours) and maintain accurate records.
- Collaborate & Report - Communicate effectively with stakeholders, attend meetings and trainings, and fulfill mandated reporter responsibilities in accordance with policies and laws
What We're Looking:
- Bachelor's degree from an accredited institution in Social Work, Psychology, Sociology, or a related field.
- Minimum of one (1) year of experience working with adults with significant mental health needs.
- Ability to communicate effectively and work collaboratively with adults, families, and team members to implement problem-solving strategies and provide ethical, respectful, and responsible services, including crisis intervention.
- Demonstrated cultural competency and ability to work respectfully with individuals from diverse cultural, linguistic, and economic backgrounds.
- Strong oral and written communication skills; ability to clearly and accurately document all interventions and contacts.
- Excellent critical thinking and problem-solving skills, including the ability to assess situations and implement appropriate solutions.
- Strong organizational skills with the ability to manage multiple priorities effectively.
- Proficiency in Microsoft Office Suite.
- Valid Wisconsin driver's license, reliable transportation, and state-minimum auto insurance; must meet organizational driving standards.
- Ability to work flexible hours, including evenings and weekends, based on program needs.
- Must successfully pass all required background checks, including vaccination requirements.
Day-to-Day work Setting: Work is performed in both field and office environments, with regular local travel and occasional statewide travel required. Flexible hours are necessary to meet consumer and program needs. Physical Demands: Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. Attendance and punctuality are essential. The role requires regular driving, standing, sitting, walking, bending, and reaching. Frequent talking, hearing, and use of hands/finger dexterity are required. Occasional lifting (e.g., files) may be necessary.
