Responsibilities: *Responsible for walk in and telephone triage.*Coordinates patients care through collaboration with the patient/family and healthcare team. *Provides patient care by assessing, planning, implementing, evaluating and documenting individual patient needs, actions taken and patient responses. *Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc. *Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task. *Documentation within scope of practice. *Performs other functions as required. Qualifications: Licensure: State of New Mexico or Compact State Nursing License or Graduate Nurse Permit requiredAcademic Preparation: *Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO) *Minimum ADN preparation with expectation of BSN completion within 5 years of hire * BLS required UPDATED 4/7/25 We're all about well-being, starting with yours.
