Responsibilities: Presbyterian is seeking a Peer Support Specialist, in this role, you will provide patientcentered recovery support by coordinating with hospital staff, funders, and community partners to ensure seamless continuity of care. I conduct patient assessments, deliver Motivational Interviewing-based engagement, and provide Naloxone education in collaboration with medical and nursing teams. I manage patient logistics including transportation, followup appointments, and discharge planning, while maintaining consistent postdischarge contact to support longterm recovery outcomes. I also create reports, presentations, and documentation, track patient progress and outcomes, and ensure compliance with hospital, state, federal, and funding regulations. Drawing on lived experience in recovery, I build meaningful, trustbased relationships that empower patients and model successful recovery pathways. I help patients access essential community resources, including healthcare, housing, employment support, and social services, ensuring a smooth transition back into the community. I collaborate with multidisciplinary teams to create a supportive recovery environment, assist with social and therapeutic activities, and strengthen patient engagement. Through mentorship, advocacy, and resource coordination, I help individuals navigate barriers, develop recovery skills, and build a sustainable foundation for longterm wellness. This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.) Qualifications:
