SUMMARY The Social Worker I provides direct psychosocial care, discharge planning, and care coordination in collaboration with patients, families, case managers, physicians, and the interdisciplinary team. Responsibilities include psychosocial assessments, identification of barriers to care, development of treatment and transition plans, patient/family education, resource referral, and clinical interventions to support safe, effective patient outcomes. In the Community Psychiatry Program (CPP), the Social Worker I also provides direct psychotherapy, crisis intervention, grief counseling, abuse reporting support, and medication monitoring in coordination with the physician and care team. QUALIFICATIONS Education: MSW required Skills/Abilities:
Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
