HouseCalls Advanced Practice Clinician (NP/PA) New Mexico \| Optum Home & Community Care Join Optum's HouseCalls team in a field-based Advanced Practice Clinician role delivering annual in-home health assessments to Medicare Advantage and other health plan members throughout New Mexico. This position focuses on preventive care, health education, care gap closure, and collaboration with primary care providers to improve member outcomes. Clinicians work independently in the community while being supported within an interdisciplinary care model. Position Highlights Conduct comprehensive annual in-home health assessments for Medicare Advantage members Statewide New Mexico territory with travel expenses fully covered Non-prescribing role focused on preventive care, education, and care coordination Base salary plus incentives, with $40,000 student loan repayment or $25,000 sign-on bonus opportunities available Required Qualifications Nurse Practitioner with active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care) or Physician Assistant with NCCPA certification Active, unrestricted state licensure (or ability to obtain by start date) Active, unrestricted driver's license and reliable transportation Ability to perform in-home assessments and navigate varied home environments Preferred Qualifications 1+ years of clinical experience, preferably in family practice, geriatrics, home health, or related settings Experience using electronic medical record (EMR) systems and technology Strong communication skills with Medicare and geriatric populations Fluency in a language other than English Ability to transport equipment up to 30 pounds and navigate stairs during home visits Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
