I. Major Responsibilities:
1. Provides in-home physical therapy and case management, including day, evening, weekend, and holiday coverage, as determined by patient and agency needs.
2. Conducts comprehensive patient assessments and analyzes data to determine diagnoses and care needs.
3. Develops and implements individualized physical therapy plans of care in collaboration with patients, families, and caregivers.
4. Coordinates care delivery and employs strategies to promote health, comfort, and safety.
5. Evaluates patient progress and adjusts physical therapy treatment plans to achieve optimal outcomes.
6. Documents all assessments, plans, interventions, and progress accurately and promptly.
7. Educates patients and families on physical therapy procedures and home care strategies.
8. Provides direction and guidance to physical therapy assistants.
9. Meets agency productivity standards for patient care.
10. Maintains current knowledge and competency in physical therapy best practices.
11. Ensures compliance with professional standards, regulations, and ethical principles.
12. Collaborates with peers and contributes to team development.
13. Integrates evidence-based practices and research findings into physical therapy services.

Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures, and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
6. Maintains, regular, reliable, and predictable attendance.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Bachelor's Degree in Physical Therapy.
2. Current Massachusetts Physical Therapist license.
3. Valid and unrestricted driver's license and access to a reliable, insured vehicle.
Preferred:
1. Master's or Doctorate in Physical Therapy.
2. Board Certification, Specialty Certification, or Case Management certification.
3. Completion of the Certified Oasis Specialist certification.

Experience/Skills:
Required:
1. Minimum two (2) years of recent (within the past 18 months) related experience either in an acute or sub-acute setting, or equivalent experience in a certified home health agency or licensed hospice.
2. Skilled in conducting comprehensive physical and psychosocial assessments.
3. Ability to work independently and manage care autonomously.
4. Strong professional communication skills with diverse patient populations.
5. Ability to perform all duties in accordance with the American Physical Therapy Association's Scop & Standards of Practice and agency policies.
6. Ability to utilize or learn various software and wireless applications.
7. Ability to read, write, and speak clearly in English.

Preferred:
1. Previous case management or program management experience.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
